When I was a pre-registration pharmacist, a nine year old girl was given methadone instead of trimethoprim. She went into respiratory depression and might have passed away had she been given a second dose.
Then later that year, a visually-impaired elderly woman was given high-dose morphine instead of her regular medication. She died.
A report by the NPA in 2018 showed that 5% of dispensing errors happened at the delivery stage, some of these being accidental deliveries of CDs.
So, after carrying out surveys over the course of a year, I designed and created a bag to transport controlled drugs from the pharmacy to the patient, no matter how short or long that step may be.
Now, the CD bag is part of protocols and policies in GP surgeries, pharmacies and hospitals all over the UK.
